(The exhibit fee check will be returned to artists not selected
to exhibit.) |

Four 35mm color slides of work done within past three years (no
glass or metal mount slides will be accepted) one booth slide. Indicate
TOP RIGHT CORNER of each slide and note on slide Artist's name,
medium, and size of piece pictured. Slides must reasonably represent
the quality, size, and nature of the work to be exhibited. They
will be kept until show weekend and may be used in show publicity. |

A brief synopsis of your history/work theories. (These may be
used in our program and publicity, if accepted.) |
| 5. |
An envelope - self-addressed and stamped with sufficient postage
to cover the weight of the slides. |
